Presentation

Lower right second premolar fractured beneath an old filling. The remaining tooth structure was insufficient to hold a new restoration without internal reinforcement.

The Challenge

Combining root canal treatment with post placement requires careful planning - enough filling material must be left at the root tip to maintain a seal, while enough space is created for the post to provide adequate retention.

Treatment

Root canal treatment completed over two visits. Post space prepared preserving the apical seal. Fibre post cemented and composite core built up, ready for crown preparation by the referring dentist.

Outcome

Tooth successfully saved. The referring dentist later placed a full-coverage ceramic crown providing long-term structural protection.
