Female, early 30s
Presentation
Intense spontaneous pain in a lower right molar, worse at night. The tooth roots were very close to the main nerve supplying feeling to the lower lip and chin.
The Challenge
The inferior alveolar nerve (the nerve that provides sensation to the lower lip) runs in a canal very close to the root tips of this tooth. Over-instrumentation or irritant extrusion could cause temporary or permanent numbness.
Treatment
Working length was determined precisely using electronic apex locators and confirmed radiographically. Instrumentation was kept deliberately short of the apex, and irrigation was performed gently to avoid forcing solution beyond the root tip.
Outcome
Successful treatment with complete pain relief and no nerve damage. The patient retained full sensation in the lower lip.
