Male, mid-40s
Presentation
Lower right first molar with persistent pain unresponsive to antibiotics. Referred when the GDP noted unusual root tip anatomy on X-ray.
The Challenge
The roots of this molar split into multiple branches near their tips (apical ramifications). Standard treatment risks missing these branches, leaving bacteria sealed inside.
Treatment
Under microscope guidance, all canals were traced to their full length including the apical splits. Enhanced irrigation with sodium hypochlorite and EDTA ensured thorough disinfection of the complex apical anatomy. Warm vertical compaction was used to fill all branches.
Outcome
Post-operative radiographs confirm filling material has reached all root canal branches. Symptoms fully resolved within two weeks.
