Patient

Male, late 30s

Presentation

Lower right first molar with deep decay extending close to the nerve of the tooth. Referred for root canal treatment due to the complexity of the root anatomy seen on initial X-rays.

The Challenge

The root canals showed unusual branching patterns and curvatures that made standard instrumentation approaches difficult. Multiple canal variations were present.

Treatment

CBCT scan was taken to map the canal anatomy in three dimensions before treatment began. Each canal was individually negotiated under microscope guidance using flexible NiTi rotary files to navigate the complex curves safely.

Outcome

All canals located and treated. Post-operative radiograph confirms complete obturation of the complex canal system.
