Female, late 40s
Presentation
Lower right first molar with severe apical curves visible on X-ray. Referred after the GDP assessed the case as beyond general practice scope due to root curvature.
The Challenge
Both roots curved sharply in their apical third (the bottom portion). Standard stiff instruments would straighten the curve, potentially creating a false channel or perforating the root wall.
Treatment
Flexible heat-treated NiTi rotary files were used, which follow the natural canal curvature. A glide path was established with small hand files first, then progressive rotary shaping maintained the canal's natural curve throughout.
Outcome
Canals treated to full length with anatomical curvature preserved. Post-operative X-ray shows the root filling faithfully following the curved canal paths.
