Presentation
Upper left molar with long-standing infection causing a large area of bone loss around the root tips (periapical radiolucency). Multiple visits required due to persistent drainage.
The Challenge
Extensive infection with a large periapical lesion required inter-appointment calcium hydroxide dressing to disinfect the canals over time. The tooth could not be sealed in a single visit due to the ongoing drainage.
Treatment
First visit: access and initial debridement with calcium hydroxide dressing. Subsequent visits over several months for dressing changes as infection resolved. Final obturation once the tooth was symptom-free and dry.
Outcome
Complete resolution of the periapical radiolucency over 10 months. The tooth was successfully restored and the bone healed fully around the root tips.
