Presentation
Lower right first molar with a well-fitting porcelain crown developed symptoms of irreversible pulpitis (nerve inflammation). The patient wished to preserve the existing crown.
The Challenge
Accessing the root canals required drilling through the existing porcelain crown, which risks fracturing or weakening it. The access cavity must be precisely placed to reach all canals while preserving crown integrity.
Treatment
Access was carefully planned using the pre-operative radiograph. A small, conservative access cavity was made through the crown. All canals were located, cleaned, and sealed. The access was then sealed with composite matching the crown.
Outcome
Crown preserved intact. Root canal treatment completed successfully through the crown with a minimal access cavity that was invisibly sealed.
