Female, early 50s
Presentation
Upper right first molar with spontaneous throbbing pain unresponsive to painkillers. Referred due to complexity of the root anatomy, particularly a severely curved mesiobuccal root.
The Challenge
The mesiobuccal root had a pronounced S-shaped curvature. This double curve creates a high risk of instrument separation (file breakage) and procedural errors if not managed with appropriate technique.
Treatment
CBCT imaging revealed the full extent of the curves. Using the operating microscope and a crown-down technique with flexible rotary files, each section of the curved canal was prepared progressively. All four canals were cleaned and sealed.
Outcome
Complete pain relief achieved. Radiograph confirms obturation following the complex S-curve anatomy faithfully.
