Male, late 40s
Presentation
Lower second molar causing intermittent pain and sensitivity to hot drinks. Referred for advanced assessment due to unusual root anatomy visible on the referral X-ray.
The Challenge
C-shaped canals form a continuous ribbon-like canal system instead of separate round canals. They are unpredictable, difficult to clean, and require specialised filling techniques. Occurs in approximately 8% of lower second molars.
Treatment
CBCT 3D imaging confirmed the C-shaped anatomy. Under microscope guidance, the entire canal system was meticulously cleaned using ultrasonic irrigation to reach areas that files alone cannot access. The canal was sealed using a thermoplastic filling technique to adapt to the irregular shape.
Outcome
Complete pain resolution. Post-operative radiograph shows thorough adaptation of the root filling to the complex canal shape.
