Presentation
Persistent dull ache in the upper right molar region. The referring dentist's X-ray revealed widely divergent roots requiring specialist assessment.
The Challenge
This upper first molar presented with widely divergent roots, making instrumentation particularly demanding. The pronounced separation between the mesiobuccal, distobuccal, and palatal roots required careful file management to negotiate each canal safely without procedural error.
Treatment
Under dental operating microscope magnification, all three roots were carefully instrumented using flexible NiTi rotary files to navigate the divergent anatomy. Thorough irrigation ensured disinfection of the complex canal system. Warm vertical compaction was used to obturate all canals.
Outcome
Complete resolution of symptoms. Post-operative radiographs confirmed satisfactory obturation of all canals with good adaptation to the divergent root anatomy.
