Male, early 40s
Presentation
Upper left first molar with a long-standing root canal treatment (>10 years old) now showing signs of re-infection. Intermittent swelling above the tooth and sensitivity on biting.
The Challenge
The original treatment showed voids and poor adaptation of the old filling material. The existing root filling material had to be completely removed from three canals before fresh treatment could be carried out.
Treatment
Gutta-percha removal with rotary retreatment files and solvent. All canals re-negotiated to true working length. MB2 canal located during retreatment. Full chemomechanical debridement followed by warm vertical obturation.
Outcome
Post-operative radiographs show dramatically improved canal filling with complete obturation of all canals including the previously missed MB2.
