Female, early 50s

Presentation

Upper left lateral incisor with a previous root canal treatment that had failed. Persistent tenderness and a small swelling at the gum line above the tooth.

The Challenge

CBCT 3D imaging revealed that the original treatment had completely missed a second canal in this tooth (an uncommon but recognised variant of upper lateral incisors). The untreated canal was harbouring infection.

Treatment

CBCT images guided precise access to the missed canal. Under microscope magnification, the hidden canal was located, negotiated to full length, cleaned, and sealed alongside the re-treated original canal.

Outcome

Infection cleared and the gum swelling resolved within two weeks. The CBCT confirmed complete treatment of both canals.
